Support for people with developmental disabilities — at every stage of life.

Since 1978, Transitions has helped children, youth and adults in St. Albert, Sturgeon County and northwest Edmonton live, work and play as full members of their community.

How we help

How can we help your family?

Tell us who we're supporting and we'll point you to the right program.

Early Childhood

Birth to age 6, for delays in two or more areas of development. Voluntary — no funding agreement needed.

Family Services

Children & youth to 17 on an FSCD agreement. Support for the whole family.

Behavioural

Birth to 17. One-to-one coaching for parents and caregivers.

Adult Services

Adults 18+ on PDD funding. 24/7 in-home support, or 5–40 hours a week living with family.
